Forms for Students/Users of CAPS:
- Contact Form — Email us about administrative issues, courses available for tutoring, employment, etc.
- Instructor Notification Form — Have us notify your instructor that you used CAPS services.
- Online Writing Lab (OWL) — Submit papers to CAPS tutors for help with structure, style, format, grammar, and thesis organization.
- Submit-a-Question — Submit questions to CAPS tutors for help with any subject: Biology, Chemistry, Languages, Math, Statistics, Physics, Astronomy, and more.
- Suggest a Workshop Topic — Got an idea for a workshop? Submit it here.
Evaluations for Students/Users of CAPS:
Tell us how we're doing!- Tutor Evaluation — Rate your experience with a specific CAPS tutor during a tutoring session.
- Tutoring Area Evaluation — Provide feedback on the condition of the tutoring areas.
- Writing Area Evaluation — Provide feedback on the condition of the Writing Center.
Forms for CAPS Employees:
- Class Visit Notification — Notify CAPS staff of a class visit you completed.
- Employee Self-Evaluation — Enter your mid-semester self-evaluation.
- Language Proficiency Form — Faculty assessments of potential CAPS' employees language proficiencies.
