Please fill out this questionnaire if you are faculty evaluating a potential CAPS employee's spoken and written proficiency with a foreign language.

Note: This form is identical to the paper form you may have received from your student(s). The web form may be used as a substitute to the paper form.

Therefore, we would greatly appreciate your help in evaluating her/his proficiency in this language. Please take a moment to evaluate this student and provide CAPS with the following information:
