Notify an instructor or advisor that you've been to CAPS.

If you would like us to notify an instructor or an advisor that you have been to CAPS for tutorial assistance, please provide us with the following information.

Note: Form must be filled out completely.

Notices will be sent out 3 times a semester (2 times during summer session) at the end of the following months: February, March, April (Spring); June, July (Summer); September, October, November (Fall).
